This order form is only for Registered Host Site Coordinators whose Host Sites are located in the LOCAL Philadelphia area and Share DELIVERS your Host Site order.

If you are an individual or pick up at a Satellite center, this is the wrong order form.  Please visit the main hosting page and select the proper form.

DIRECTIONS - READ BEFORE USING ORDER FORM

1. Find an item you want and click 'Buy'. Then click on OK to return to the item list. Find the next item you want and click 'Buy'. Continue this process of adding each item to the cart.
   (The quantity will be set in the View Cart section.)
   
   
2. Click 'View Cart' to see your items, set the quantity (see instructions on that page) and view the total due. To continue adding items from within the view cart, click on 'Return to Order Items' link. And, if you are ready to send us your order, click 'Check Out'.
   
   
3. Towards the bottom of the customer/Host info page you will see a box for your Host number, volunteer work and order notes.  Enter your Host Number along with any volunteer work completed and any notes you may need us to know about.
